About the Unit/Department/Team The Lakewood Social Services team is a dedicated group of professionals committed to addressing the diverse needs of our patients. This role will benefit from the collaborative efforts of various disciplines within our organization, ensuring comprehensive support. Our team is focused on helping patients navigate both internal and external resources, providing guidance and assistance every step of the way. As part of the care management team, we consist of licensed social workers, two dedicated chaplains, a physician's assistant, and a skilled group of nurses and certified medical assistants. Together, we provide holistic support to our patients, addressing their medical, emotional, and spiritual needs. Our collaborative approach ensures that each patient receives comprehensive care tailored to their individual circumstances. Mission for Position The Social Worker position will be responsible for assessing and addressing the emotional, social, and spiritual needs of patients with serious or advanced illnesses. This role will collaborate with the clinical care team to create and implement personalized care plans, support patients and families in navigating healthcare services, and act as a patient advocate. Position Summary The Social Worker position will be responsible to meet the needs of patient care for discharge planning, triage support to acute care settings throughout Lakewood Health System, as well as resource planning to our patients/caregivers and families. Key responsibilities in this role include coordinating care with interdisciplinary teams, providing patient education, and ensuring accurate documentation. Other responsibilities will includecollaboration of care with communityresources, patientcare teams, andcommunity outreach needs. Position Type This is a full-time position working 64-80 hours every two weeks, Monday through Friday.
