Full-Time (40 hours per week primarily Monday-Friday from 8am-4:30pm), Non-Exempt, Benefit & Pension eligible position Our CADI/CAC/BI Social Workers coordinate delivery of services to individuals under the age of 65 with a goal to assist people to remain living in a community setting. They connect people with community resources, monitor service delivery, help individuals develop a person-centered plan outlining what is important to them, for them and how they can be best supported. They develop positive working relationships with community partners and providers as well as collaborate effectively with work teams.
