Brown County Full Time Social Worker I - Adult Mental Health in New Ulm, Minnesota
Coordinates services and referrals for individuals and/or families. Provides assessments to determine program eligibility for need for further intervention. Provides voluntary and/or involuntary services and case management, including preventative services. Develops intensive long or short-term person-centered plans to meet individual and/or family needs. Consults and coordinates with a variety of professionals including but not limited to psychiatrists, psychologists,attorneys, medical personnel, guardians, law enforcement and other providers. Arranges for court-ordered interventions including but not limited to out of home placement, facility admission,treatment, and guardianship/conservatorships. Prepares court reports and petitions that include recommendations. Testify in civil, criminal and administrative hearings. Interprets and implements judicial findings and court orders. Assess the health and safety needs of clients. Protect client confidentiality in compliance with data privacy laws and rules. Act as a mandated reporter of child and adult protection concerns. Mobilize crisis resources to reasonably assure the health and safety of clients. Provide direct and indirect support to families and individuals in the home, office, and community. Assist with transporting clients as needed. Knowledge and ability to effectively use SSIS and other systems required to conduct job responsibilities. Maintain case records which meet federal and state guidelines. Complete significant amount of documentation required by federal and state mandates. Be knowledgeable of legislative changes and mandates. Be knowledgeable of community resources. Works closely with all other units within Brown County Human Services, the Department of Human Services and othercounty Human Services agencies across the state. Represent the county as approved on various boards and committees and special projects Perform office intake and weekend on-call duties as required by the agency. Provide community education. Other duties as assigned or apparent. Regular attendance is required. Attends meetings and complete training. Performs duties for adult mental health mental health case management and care coordination for eligible clients. Adult duties include case management for voluntary and civilly committed clients. Coordinate access to mental health diagnostic assessments for eligibility determination. Participate in on-going clinical supervision Coordinate with mental health, housing, medical, educational, and vocation providers. Monitor client compliance with terms of court orders, provisional discharges, and report to the courts, DHS, and StateForensics Services to ensure safety of client and community members. Coordinate mental health services for clients. Obtain resource information and provide to clients. Serve as members of the prepetition screening team and are responsible for deciding whether a client is referred tothe county attorney for civil commitment. Make decisions about whether clients are revoked from provisional discharges and stays of commitment. Mobilize crisis resources to reasonably assure health and safety of clients. Research to find placement options for clients, make referrals and coordinate admission. Qualifications Bachelor s Degree in Social Work, Psychology, Sociology, or other Human Services related field.
