We at Fairview Health Services are looking for a dedicated Registered Nurse/RN to join the Edina Clinic! This position is approved for 72 hours every two weeks, and is benefit eligible! Responsibilities Job Description

The RN is responsible for planning, organizing, coordinating, and providing professional nursing care for patients in an ambulatory care setting. While following established standards and protocols, the registered nurse delivers care that is unique to the needs of the ages served by the clinic. As a member of the care team, the RN supports quality patient and family-centered care principles through performing a variety of high-level functions within a care team to support the healthcare team and patients/support network. The nurse will educate patients and the public about various medical conditions, maintain complete, timely and accurate clinical documentation for each patient and continually look for opportunities to improve processes and workflow.

  • Assess patient needs and identifies expected nursing outcomes. Implement nursing interventions based on plan of care.

  • Evaluates care and the patient’s response to interventions and outcomes.

  • Delivers care as delegated by physician/authorized practitioner to assigned patient populations, or individual patient.

  • Independently triage telephone calls from clients. Using the nursing process facilitates access to the health care system and/or finds a satisfactory resolution to the clients' concern.

  • Navigate and facilitate patient care plan involving Outpatient Diagnostic Studies, Post-Clinic Responsibilities and transition along the health continuum. Provides proactive disease management using evidence based practices. Follows patients progress through phone calls, test results and report preparation.

Qualifications

Required

  • Current Minnesota RN License

Preferred

  • BLS Certification Preferred

  • 3 or more years' proven peripheral vascular disease experience, inpatient and/or outpatient
