We at Fairview Health Services are looking for a driven In Clinic Visit Facilitator to be based out of our Ridges location in Burnsville, MN; and work in the Sports Medicine Department! This position is approved for 80 hours every two weeks and is benefit eligible. This position will be supporting in clinic the Orthopedic surgeons and Sports Medicine non operative providers at our Ridges (Burnsville) and Woodwinds (Woodbury) locations. Responsibilities Job Description

In Clinic - Visit Facilitators are responsible for supporting in person clinic visits, including communicating with patients prior to their visit and helping guide patients through their care experience. The visit facilitator captures pertinent medical information from the patient and details the information into the patient’s medical record. This role also helps to ensure the exam rooms are stocked, equipment and supplies are available, and assists with tasks that support the effectiveness and efficiency of the team. Visit facilitator's may also assist in scheduling and coordinating appointments, preparing schedules and other delegated duties as assigned.

  • Prepare patients for their appointments to ensure medical records are collected, received and available for visit if applicable.

  • Assist in the rooming process by collecting and detailing pertinent information such as the reason for their visit, patient reported medication review, vitals collection with automated vitals machine and applicable health questionnaires.

  • Prepare exam rooms and stock supplies

  • Supports the patients' connection to the provider for the visit and serves as a resource ensuring a positive patient/provider experience.

  • Completes visit wrap-up ensuring the after-visit summary is provided and patients are aware of recommendations for follow up. This may include assisting with scheduling follow-up appointments.

  • Communicates successfully with patients, providers and other health professionals, demonstrating comfort in person and with virtual technologies.

Qualifications

Required

  • At least 1-year of customer service experience

  • Proficient with technology (computer hardware and programs, smart devices such as phones or tablets)

Preferred

  • Current nursing, medical, social work, psychology, or pharmacy student

  • Successful completion of, Nursing Assistant, Emergency Medical Technician, Heath Unit Coordinator program.

  • Medical terminology

  • Epic Health Records

  • Telephonic customer experience

  • Previously worked in a healthcare setting
