Responsibilities The PrairieCare Inpatient Social Worker (LGSW required/LICSW preferred) carries a patient caseload at the 101 bed Psychiatric Hospital in Brooklyn Park, Minnesota, serving ages primarily 5-18 and adults 18-35. The Social Worker focuses on facilitating groups, treatment planning, discharge planning, care coordination, communication with caretakers, community providers, and others involved in the care of patients. Social Workers collaborate daily with the Child & Adolescent Psychiatrists and Therapists on the patient's primary Treatment Team to advance the plan of care within the average patient length of stay, 7-10 days. Accountable to uphold and apply clinical knowledge and understanding of emotional and behavioral health practices and principles Responsible to assess, plan, implement, and evaluate the psychosocial services for patients and share social work expertise with members of the Treatment team within the average length of stay of 7-10 days Develop and revise treatment and complex discharge plans and communicate these with patients, caretakers, the treatment team, and direct care staff Provide psychoeducation to patients, caretakers, the treatment team, and direct care staff regarding diagnosis, symptom management, recommendations, discharge plans (including leading discharge meetings), safety plans, patient rights, financial impact issues, and other pertinent patient care matters Communicate effectively with external providers, agencies, and government and private payer networks to develop relationships and refer patients to the next appropriate levels of care Responsible for leading child and adolescent therapy/process groups on the inpatient units Be oriented to ethically provide and advocate for patient-centered, culturally appropriate, age-appropriate care Educational Opportunities: Continuing Education offerings monthly through various clinical consultations and the PrairieCare Education Series Development opportunities within the PrairieCare Mentorship Program, Leadership Development Program, and more! Collaboration with interdisciplinary treatment teams who are experts in their respective fields Hours: PRN role - minimum of 4 shifts per month Qualifications Master's Degree in Social Work required- applications welcome within 3 months of graduation! Current Minnesota Board of Social Work Un-Restricted License required (LGSW required or LICSW preferred) -Contingency offers possible if License is pending/in progress- please apply today! Two years of experience in a mental health setting preferred Within the training process, complete Think: Kids: Collaborative Problem Solving (CPS) training and recognize that Kids Do Well If They Can. Social Workers participate in using the CPS Assessment and Planning tool to help kids develop skills for their challenging behaviors. Experience working with children, adolescents and their families strongly preferred Experience using electronic health records preferred Knowledge of Microsoft Office products including Word, Outlook, and Teams Ability to communicate in written and verbal form effectively and efficiently Ability to coordinate care with elevant staff Ability to shift between dissimilar tasks smoothly and rapidly Ability to make effective decisions while in stressful situations and circumstances Bilingual and Multilingual Candidates are strongly encouraged to apply.
