Lifeworks Services Inc Direct Support Professional 2 (DSP in Apple Valley) in Apple Valley, Minnesota

Lifeworks is seeking a full-time Direct Support Professional 2 to work with adults with disabilities in the Apple Valley facility, the surrounding community and virtually. The Service Facilitator position will support individuals through focused activities that challenge and enrich the long-range goals of the individuals served. Direct Support Professional 2 carry a caseload; attend intakes and annual meetings; complete annual paperwork; and coordinate with the individuals served and their support teams to ensure person centered planning and services. How you ll be spending your time: Work closely with program team to meet the daily needs of individual served Identify, write, and implement new person-centered plans and goals for the individuals. Effectively communication and coordinate with individuals support teams. Organize group activities in the community or facilities based on the interests of the members of the group. Transport individuals out into the community for volunteering and other outings using a company vehicle. Ensure that individuals health and safety is maintained by following their individual plan of care and Lifeworks policies and procedures.
